Smart Tuition Form
All families are required to return their completed Smart Tuition form and Tuition Agreement and Policy to the school office by June 13, 2016. Contact the school office if you need an additional copy.
As their third trimester project, grades 6 and 7 are producing a video about our school, its people, and what makes us unique. They've decided a student-created song would be just the thing to make the video complete.
So here are the rules!
-Your song should celebrate and encourage school spirit!
-Open to Grades 1-8
-Entry may not be longer than three minutes
-All themes, language, etc. must be appropriate for a Catholic school setting, of course.
-You may use instrumental/background music from existing songs if you like.
Audio recordings are due to Ms. Campanella at firstname.lastname@example.org by May 6, 2016.
Good luck to all and we can't wait to hear what you can do!
OLGCA parents and students: would you be so kind as to go to the link below and fill out this quick survey about our LIbrary? If you got the OptionC alert email, this is the same link. Please copy and paste the link if it will not click through for you.
We would like to hear from as many parents and students as possible, but please don't fill it out more than once per person.
Thank you very much for your help! We continue to move forward to better and better things!

One more day to donate to Operation Gratitude!
This morning, a reporter and cameraman from The Tablet and Currents on CTN came to do a story about OLGCA and our drive for toiletry and personal items for Operation Gratitude. (The reporter saw it on our school calendar online and called to ask me about it.) They interviewed Ms. Donnelly, our Student Council officers, and me about what prompted the drive and what we hoped to accomplish by it.
It is going to air tonight on CTN at 7:30 and 11:30, and again at 6:30 am tomorrow.

Parents of Nursery students from OLG who graduated in June should either have been contacted by DOE or will be contacted soon.
Parents with UPK-eligible children, who did not attend Nursery here, who would like to enroll in the PreK program being held at this site should visit https://maps.nyc.gov/upk/ and click the "Call Me" button to have a representative contact you.
Or, call 212-637-8000 to speak directly to someone at the Department of Ed. The proper name is "PreK Center at 385 Avenue W" and the program code is 21Z077.
Please keep in mind that although PreK is being held at our site, it is not being conducted by Our Lady of Grace. Therefore all registration has to go through the Dept. of Education, and PreK students will follow the public school calendar.
Thank you for your continued patience with this process.

All students entering 6th grade are required to have a Tdap vaccine, two doses of chickenpox vaccine and a dose of Polio vaccine that was given after 4th birthday.
All students entering 7th grade are required to have a meningococcal vaccine.
The forms with required vaccines were issued in school. If you received the form, either your child requires the shot or the nurse doesn't have a record of the particular shot. Please contact your child's medical provider regarding the shots and send all the necessary documentation (a copy of the immunization record or doctor's note with the date of administration) to school.
Students who had medication in school last school year were given new forms. If your child needs medication during next school year, bring completed forms as soon as possible.
Students are not allowed to bring any medication to school and the Nurse cannot administer any medication without proper paperwork. If your child needs to take medication while in school either daily or as necessary, please contact school nurse for a special form that the doctor has to fill out. You can also print those forms from Department of Education website, Office of School Health page, Health Services/Section 504 Accommodation section.
Call school nurse if you have any questions at 718-375-2081
